About BioMarin RareConnections™

Our Case Managers are ready to provide guidance on how your patients can gain access to BioMarin therapies by:

  • Conducting benefits investigations
  • Helping your patients understand insurance coverage and financial assistance options
  • Helping to identify prior authorization requirements for individual patient plans
  • Working with your patients or their caregivers to coordinate delivery or administration of BioMarin therapies
  • Providing ongoing product support of BioMarin therapies

For a patient to be enrolled in BioMarin RareConnections, he or she must first have a valid prescription for a BioMarin treatment. You can start by completing the Patient Enrollment Form, which serves as the prescription.

Patients must complete the BioMarin RareConnections Patient Authorization Form (PAF) in order to receive support from BioMarin RareConnections.

Use this form to start enrollment in BioMarin RareConnections. This form serves as the prescription for your patient's KUVAN® (sapropterin dihydrochloride) and PALYNZIQ® (pegvaliase-pqpz) treatment.

Patients must complete the BioMarin RareConnections Patient Authorization Form (PAF) in order to receive support from BioMarin RareConnections. Through this form, patients grant BioMarin permission to use and disclose their health information in order to help them access BioMarin therapies. Your patients will need to provide consent to both BioMarin and you, their healthcare provider:

  • You, as their healthcare provider, need written consent from your patients to release their protected health information (PHI) to BioMarin
  • BioMarin also needs your patients’ written consent to share their information (including genetic information) with other providers to assist them with accessing services that support their treatment

Once your patients give consent, BioMarin RareConnections will help your patients work with their insurance provider to obtain reimbursement for their care and will help them with other case management services.

Once your patient is enrolled in BioMarin RareConnections, his or her dedicated Case Manager will reach out with an introduction and contact information. This Case Manager will be your patient’s ongoing partner, providing product support services to help your patient start and stay on treatment.
