Complete the Patient Consent Form (PCF), which authorizes your doctor and health insurance to share your information with BioMarin.

Your BioMarin Clinical Coordinator will reach out to get you started.

2.

Ask your doctor to complete and submit the Patient Enrollment Form (PEF).

Once the forms have been submitted, your BioMarin RareConnections Therapy Access Manager (TAM) will reach out about coverage and options to help you pay for your testing and treatment.
